TELEHEALTH NURSE (Cardiac, Respiratory and Diabetes) – TRURO

Hays Healthcare in conjunction with a major client, are looking for nurses (Band 5 and above) to work within the Telehealth sector in Truro, Cornwall. This is a fantastic opportunity within a role and service that has massive potential for personal and career development.
Role Summary:
As a Telehealth Nurse you will assess the data of Telehealth patients, with the assistance of decision support software. Nurses will then provide advice as appropriate in compliance with associated protocols.
Job Skills:
· To have a patient centred approach to care
· To provide clinical assessment and triage to patients living with one or more long-term conditions aided by decision support software (Clinical User Interface).
· To be clinically competent in the management of one or more long term conditions (COPD, Chronic Heart Failure, Diabetes)
· Accurately triage and prioritise patients.
· To respond to alerts determined by clinically approved predefined protocols
· To review regular reports and patient analysis.
To apply for this position, nurses should have the following qualifications and experience:
· NMC Registered General Nurse (3 years experience desirable)
· Additional qualification in the management of a relevant long term condition – COPD, Chronic Heart Failure or Diabetes
· Evidence of continuing post-registration development in a relevant long term condition
· Experience within COPD and Respiratory medicine
· Good IT skills
· Confident decision maker and effective team player
· Ability to meet targets
