TFC Program Coordinator (M-F 9-5p)

NHS Human Services is currently seeking a full-time Program Coordinator in its DuBois, PA location to provide clinical oversight to the children on their caseload. Weekly therapy with the child and bi-weekly therapy with the family is provided. Support and strategies are given to the Host families.
The person in this position is responsible for ensuring the execution of all placement activities for the designated caseload of up to 12 children, placed within the specific site. The Program Coordinator is also responsible for monitoring all activities of the caseload to include intake; service planning; health planning'; education and vocational planning; educational and vocational planning; safety planning and permanency planning, all withing agency and state standards and regulation.
The Program Coordinator is responsible for activities described in the Program Description, including case coordination, skill building interventions with youth, monitoring the child's progress, implementing interventions from the treatment plan and/or ISP, and assisting with supporting and training Host Home Parents. In collaboration with other team members, the Program Coordinator monitors the Therapeutic Family Care parents to assess stressors in the home, provide technical assistance to the families to insure adherence to the treatment plan and/or ISP.
Program Coordinators are expected to participate in on-call responsibilities, as shared in a rotation with other direct care staff (Program Manager, Coordinators, and Specialists) within the local site office.
General Responsibilities:
- They will meet regularly with the TFC parents to develop strategies for meeting the youth's needs in the home.
- Supervise interventions provided by the TFC parents and assist the Program Coordinator in the oversight of the treatment provided in the host home.
- Provide family therapy to the biological family.
- Design and direct the implementation of a comprehensive plan that provides goals and strategies for ensuring the execution of all placement activities.
- Be responsible for a designated caseload of up to 12 children placed in the Therapeutic Family care program.
- Identify behavioral goals and intervention techniques.
- Recommend non-aversive behavioral change methods.

Job Requirements:
Qualified candidates will possess the following:
- Bachelor's Degree in a mental health field, such as psychology, counseling, or social work.
- Two (2) years of direct care or case management experience.
- Valid Pennsylvania Driver's license and use of personal auto during working hours is required.
- Strong verbal and written communication skills, positive attitude and the ability to work well with a team.
