**The Master's level Social Worker Therapist provides direct care for adult patients with psychiatric and substance abuse conditions. Responsible for collaborating with the patient, family, multi-disciplinary team and community agencies to support and coordinate treatment and discharge planning. Responsible for continuing own personal development to enhance practice. Performs other duties as assigned. Works with adult patients, age 18 years and older.

PRIMARY JOB DUTIES:

1. Completes psycho-social assessment of patient within established time frames and either face to face or telephonically, with all patients family or significant other when appropriate, to determine needs.

2. Collaborates with physicians and other health team members in developing the patient plan of care based on on-going patient assessment. Provides individual, group and family therapy.

3. Annually continues personal development to enhance practice and profession.

4. Appropriately communicates information in accordance with hospital policies and procedures.

5. Organizes and performs work effectively and efficiently.

6. Collaborates with physicians and other team members to integrate plan of care.

7. Demonstrates positive interpersonal relations in dealing with all members of the team.

8. Maintains and promotes customer satisfaction.

9. Effectively demonstrates the mission, vision, and values of the medical center on a daily basis.

10. Performs other duties as assigned.

ESSENTIAL JOB SPECIFICATIONS:

1. Education: Masters in Social Work or a human services related field.

2. Licensure / Certifications: License to practice from a school in the State of North Carolina for Social Work. Professional Counselor or Psychologist or Rehab Therapist or eligible to be licensed.

3. Experience: Experience preferred.

4. Essential Technical/Motor Skills: Hand / Eye coordination in order to use computer, etc. The ability to speak clearly, to communicate with patients, families, physicians and staff regarding patients' condition and care. Must be able to communicate and be literate in the English language.

5. Interpersonal Skills: Ability to develop positive interaction with patients, patients' families, physicians and staff in order to effectively care for the patients.

6. Essential Physical Requirements: Must have full mobility, with the ability to participate in vigorous physical activity, which may included (but not limited to), bending, stooping, twisting, supporting, lifting (maximum of 45 lbs.), and moving patients in a controlled, safe manner. Must be able to complete all physical requirements of current NCI standards.

7. Essential Mental Abilities: Must be able to assess a patient's condition, formulate a plan of care, select appropriate interventions, evaluate patient's response to care / treatment, and to explain / teach patients about their condition / recovery. Requires higher level of mental faculties accompanied by short-and long-term memory.

8. Essential Sensory Requirements: Ability to visually assess patients and to utilize sight to implement and evaluate plan of care.

9. Exposure to Hazards: Frequent exposure to blood and body fluids, infectious diseases, needle puncture wounds, occasional exposure to radiation in certain areas. Potential for physical harm.

10. Other - Hours of Work: Flexible schedule to accommodate staffing needs which may include weekends, holidays, rotating shifts and on call.

11. Population served: Neonates Infants Pediatrics Adolescents (12 - 17 years) Adults (18 - 71 years) Geriatrics ( 72 + years)
