Position Summary:

Thrombosis Service Supervisor:
The incumbent supervises and coordinates 24/7 daily operations, pharmacy programs and activities to include recruiting, hiring and scheduling staff, conducting performance evaluations, conflict resolution and program development. This position provides the essential functions of a Clinical Pharmacist.
The Thrombosis Service consists of three main areas, 2 pharmacy run clinics and a nurse hospital team, that work together to provide care for more than 90% of anticoagulated patients in the University of Utah Health Care system. This position also serves as part of the Thrombosis Service leadership team in guiding the goals, mission, and vision for moving the practice forward, optimizing patient safety, assuring compliance with standards of care, promoting continuity of care, reporting quality measures, and fostering a team atmosphere. Shared call is required.

Essential Functions:

Provides the essential functions of a Clinical Pharmacist.
Coordinates and supervises pharmacy programs to include recruiting, hiring, scheduling staff, performance evaluations, conflict resolution and program development.
Collaborates with staff and groups to effectively implement and maintain pharmacy programs.
Evaluates all operations to include workload measures, utilization of resources and productivity.
Develops policies and procedures to ensure department operations meet divisional, departmental and hospital goals and mission.
Participates in departmental budget development.
Identifies and coordinates staff development programs.
Coordinates educational opportunities for students, residents, and other department trainees.
Participates in and takes a leadership role when needed in organizational committees and meetings.

Knowledge / Skills / Abilities:

Ability to perform the essential function of the job as outlined in the position description.
Ability and willingness to train department trainees (eg students, residents, new staff).
Ability to assess data regarding the patient’s status and provide care as described in the department’s policies and procedures manual.
Demonstrated authority to make personnel decisions including hiring, staffing, adapting schedules to departmental needs, and taking disciplinary actions where warranted.
Demonstrated the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.
Demonstrated knowledge of the principles of life span growth and development and the ability to assess data regarding the patient's status and provide care as described in the department's policies and procedures manual.

Qualifications

Qualifications:

Required:

Licensure to practice pharmacy and to dispense controlled substances in the State of Utah.
Advanced degree (PharmD or MS) or equivalent clinical practice experience.
Demonstrated effective written and oral communication skills, service orientation, and organizational skills.

Preferred:

Previous supervisor experience and residency training.
