Principle Duties and Responsibilities:

The full-time Transition Specialist will have administrative / clinical responsibility for coordinating inmate transition planning. These responsibilities include:
      Coordination of offenders’ re-entry for both pre/post release services.
      Facilitation of Phase 4 re-entry groups.
      Preparation / Submission of pre/post release dockets to DOC classifications.
      Monitor completion of Re-Entry Preparation Activity Checklist (REPAC) by offenders.
      Distribute REPAC to Probation and Parole District and or community treatment programs.

Job Specifications:

The Transition Specialist will be credentialed as per the requirements of the VA Department of Health Professions 18VAC 115-60 et.  Seq. Regulations Governing the Certification of Substance Abuse Counselor or in the process of becoming certified and eligible for certification in two years. Minimum BA degree, CSAC certification preferred and experience with substance abusing populations and knowledge of therapeutic community are required. A Master’s degree is preferred; however, relevant experience and / or training may be substituted.
 
Additional professional skills must include verbal and written communication skills, organizational skills, and ability to work effectively with staff, correctional personnel, and clients. He/she must have the ability to adapt to work effectively with staff and knowledge-to a correctional setting, support implementation of the Community Education Centers’ (CEC) therapeutic community treatment program within a correctional environment and provide transition planning and placement services for client reintegration to the community during re-entry and post release phases of the program.
