FRAMINGHAM, MA

Long-Term and Short-Term Care Rehab Facility St. Patrick’s Manor, in support of its mission and philosophy, has embarked
on the development and implementation of a designated Transitional Care
Unit and is seeking a qualified individual to manage the unit.

Transitional Care Unit Director

The ideal candidate will have demonstrated, proven successes in progressive
nursing leadership roles within the Long Term Care setting. Applicants must
possess the skills and experience necessary to actively participate in the
implementation phase of the new transitional care unit. Requires recent working
knowledge of federal, state and Medicare skilled care regulations; ability to
work in a fast paced environment and be detail oriented; and experience leading
a successful, multi-disciplinary team. Experience with a clinical computer system
desired but not required. This position reports jointly to the Director of Nursing
and Assistant Administrator.

physicians, and other professional caregivers at the Manor, providing the best
possible experience for our Residents and their families.
