Visiting Nurse - Hospice

The Visiting Nurse for Home Health and Hospice provides comprehensive physical and psychosocial assessment and nursing care planning for home health/hospice patients in the home setting including pain and symptom management. You will identify problems, counsel, and teach patients, family and caregivers appropriate nursing care skills; perform skilled procedures to include injections, complex wound care, catheter insertion and care, colostomy and tracheostomy, central line IV care and blood draw; and maintain accurate/current patient records for purposes of continuity of care, team communication and program quality evaluation. Care and case management will be handled with a talented interdisciplinary team. You will be accountable for assessment, planning, interventions and evaluation for the duration of contact with the patient to include responsibility for team scheduling coordination.
This position will focus on Hospice nursing for the eastside south Home Health Hospice team.
Education
Minimum Education Requirements
Bachelors of Science in Nursing or related healthcare 4-year degree
Vocational School/Certificate Prog Graduate of accredited RN program
Knowledge/Skills
Minimum Knowledge/Skills Requirements
Ability to work independently. Ability to transfer / position patients as needs require. Basic laptop skills including MS Office Suite. Customer service skills. Demonstrated experience providing care planning within a community setting (continuum of care).
Preferred Knowledge/Skills
Horizon for homecare system experience.
Experience
Minimum Experience Requirements
2 years Current medical surgical or home health experience.
Preferred Experience
2 years Home health/hospice experience.
License/Certifications
Minimum License/Certifications Requirements
At time of hire Current Washington State RN License. Washington State drivers license with proof of insurance.
