UIC - DSCC needs a Care Coordinator, civil service title, Medical Social Consultant, to coordinate care for eligible children, birth to 21, with special health care needs. The minimum starting annual salary is $39,078. Hours are M-F 8-4:30. Minimum qualifications are a MSW and 3 years post degree professional experience including 1 year in a clinical or medical setting. Pediatric experience is beneficial.
