Learn more at www.multicarejobs.org Position Summary: The Care Manager is a Registered Nurse responsible for the clinical, quality, and cost effective outcomes for a caseload of patients. He/She facilitates coordination of an interdisciplinary plan of care to guide patient progress toward achieving clinical, quality, and cost effective outcomes. The Care Manager mobilizes resources and manages the system in response to patient needs according to an interdisciplinary plan of care. The Care Manager, as an expert clinical practitioner, participates in the development and evaluation of clinical guidelines or standards of care for a designated population of patients. She/he collects, trends, and analyzes aggregate outcomes data for that population to be used in identifying best practices and targeting performance improvement efforts. There may be a variation related to the Careline specification. The Care Manager is certified, or eligible to become certified, in Case Management, Outcomes Management, or Healthcare Quality. Minimum Requirements: Three years recent clinical experience in area of specialty preferred. Education/Licenses/Certifications: BSN and/or Baccalaureate level education required. Masters degree in nursing preferred. Current certification in specialty field preferred. Case Manager Certification, highly desirable.
