CHC Case Manager- Brain Injury

Hays Healthcare is urgently requiring a Band 6 (RGN, RMN,RSCN,RNLD) Continuing Healthcare Case Manager specialising in Acquired Brain Injury and Complex Care to work with one of our long-standing clients in Leicestershire. The position required will form part of the Continuing Healthcare division for an initial period of 3 months with the possibility of extension. This role is offered on a full time basis, 37.5 hours per week, Monday- Friday.

The main responsibilities of this role include:
• Managing a defined caseload of Complex Cases and Acquired Brain Injury Cases
• Working alongside the Brain Injury Unit and the team at the hospital to ensure that the best level of support and case management is provided to patients with an acquired brain injury
• Providing families and carers with information about brain injury and sources for support
• Providing Clinical Supervision to Band 5 Nurse Assessors
• Coordinating and facilitating Multi Disciplinary assessments and patient reviews
• Ensuring that care plans are produced to meet assessed care needs of the patient
• Coordinating and facilitating timely reviews for patients who have care packages in place

The ideal candidate will have:-
• Previous experience as a Continuing Healthcare Nurse Assessor (desirable)
• Knowledge of the Department of Health National Framework for NHS Continuing Healthcare.
• Knowledge and experience of patients with Acquired Brain Injury or Complex Cases
• Experience of making assessments against identified standards
• Knowledge and experience of risk assessments, clinical governance, policy and audits
• The ability to build strong relationships with clients
