Hays Healthcare is looking for a Band 6 Registered General health nurse to work with our client based in Wakefield. The role will require you to identify individuals who frequently call the 111 or 999 service and determine the correct forms of care needed to help the individuals.
You will be required to:
- To review and manage in partnership with key stakeholders frequent callers.
- Reduce overall number of people identified as frequent callers and setup a system for continued management on a day-to-day basis.
- Working alongside the clinical pathways managers to identify pathways more appropriate for these patients
- Provide weekly updates on progress and to create a monthly report for the EOC management Team
- Promote the initiatives to reduce inappropriate callers with both internal and external stakeholders.
This role is to work Monday – Friday full time agency, the role is initially until March 2015 but may be extended.
