Full-time Nurse Practitioner – INSPIRIS

Responsibilities for Nurse Practitioners include:
-Providing direct care to aging residents in long-term care facilities
-Collaborating with primary care physicians, facility staff and patients' families
-Participating in care conferences with other providers and patient family members
-Working with utilization review process for each facility
-Identifying patients' behavioral health issues
-Educating nursing home staff, physicians and administration
-Coordination of care for patients

Positions in this function require license and certification as Nurse Practitioner, Physician Assistant, or Clinical Nurse Specialist. Provides primary case management of chronic and acute conditions.

Requires current unrestricted NP licensure in applicable state.
Requirements:
-National Nurse Practitioner certification
-Certification in Adult, Family or Gerontology
-Licensure in the State where this position is based
-Intermediate level of proficiency in PC-based word processing including basic Microsoft Word and Outlook skills
Additional Preferred Assets:
-Clinical experience
-Long-term care experience
-Knowledge of mental health issues
-A background in managed care
-Familiarity with Medicare and ICD-9 coding
