Job Purpose:
Enables clients to stay in theirhomes by monitoring and recording client's condition; providing support andpersonal services.
Duties:
* Monitors individual's conditionby observing physical and mental condition, intake and output.
* Supports individual byproviding housekeeping and laundry services; shopping for food and otherhousehold requirements; preparing and serving meals and snacks.
* Assists individual by providingpersonal services, such as, bathing, dressing, and grooming.
* Helps individuals care forthemselves by teaching use of cane or walker, special utensils to eat, specialtechniques and equipment for personal hygiene.
* Records individuals informationby making entries in the client's documentation; notifying nursing supervisor ofchanging or unusual conditions.
* Maintains a safe, secure, andhealthy environment by following all company standards and procedures;maintaining security precautions; following prescribed dietary requirements andnutrition standards;
* Updates job knowledge byparticipating in educational opportunities.
* Protects the home care agency byadhering to professional standards, home care policies and procedures, federal,state, and local requirements.
* Enhances service reputation by acceptingownership for accomplishing new and different requests; exploring opportunitiesto add value to job accomplishments.
