Job Description: The Home Health Aide/Hospice Aide is a trained person who provides personal care and assistance in activities of daily living for people in their place of residence. These services are included in a written plan of care prepared by a health care professional and signed by a physician. The Home Health Aide/Hospice Aide works under the direct supervision of a Registered Nurse and/or when appropriate a Physical, Occupational or Speech Therapist under the delegation of their License. Basic Qualifications: High School Graduate or GED equivalence preferred. Meets Federal and State guidelines required to work as a Home Health Aide in a certified licensed Home Health agency. Has worked as a Certified Nursing Assistant within the past 24 months preferred. 18 years or older preferred. Reliable transportation, the ability to transport patients and acceptable driving record as defined by policy #2250 ?Motor Vehicle Requirements?. Other Qualifications: Ability to read and communicate effectively in English. Basic computer skills and experience with other technological devices. (i.e. cell phone, black berries, etc?). Ability to work irregular hours (days, evenings, holidays, weekends). Self motivated. Able and willing to drive in all weather conditions. Able and willing to work with all populations served. License or Certification: Certified Nursing Assistant, on State of Wisconsin Registry. Current CPR certification or the ability to be certified prior to start date. Valid driver?s license. Benefit Eligible: Yes Exemption Status: Non-Exempt Hours/Pay Period: 48 hours per pay period Schedule Details: Days and hours vary Weekend Schedule: Weekends vary Compensation Detail: Education, experience, and tenure may be considered along with internal equity when job offers are extended.
