 In this position, the incumbent will:

 

Travel to patient homes to implement and collaborate with the plan of care.

 

Understand and assure regulatory compliance issues.

 

Demonstrate efficient and effective use of available resources.

 

Serve as a role model and practice resource for agency staff.

 

Monitor and evaluates patient care outcomes.

 

Demonstrate expertise in the delivery of patient care.

Bachelor's degree in physical therapy from an accredited university

Minimum one year clinical experience preferred

Licensed as a physical therapist by the State of Wisconsin

CPR certification

Must have the ability to connect to the internet with company issued laptop from home using dialup or hi speed connection (DSL, cable, or satellite).

Valid driver's license, use of car and proof of auto insurance required
