MDS Coordinators – RN or LPN
About Us:
Sprenger Health Care Systems is a family owned organization that has provided quality rehabilitation and nursing services for over 50 years. We operate a network of 10 health care communities throughout Lorain, Summit, Wayne Stark counties.
We currently have openings for MDS Coordinators at the following facilities:
- Rose Lane Nursing Rehabilitation in Massillon, Ohio
- Anchor Lodge Retirement Village in Lorain, Ohio
Responsibilities:
- Coordinate the RAI process for all assigned residents according to state and federal regulations.
- Schedule all OBRA required and PPS assessments according to state and federal regulations.
- Complete and track all admissions, discharges and re-entries to the facility.
- Schedule and communicate Plan of Care Meeting for all residents.
- Oversee the interdisciplinary team in regard to MDS, RAP and care plan activities for assigned residents.
Qualifications:
The successful candidate must be an Ohio RN or LPN with preferred experience in MDS and long term care. Also necessary is excellent organizational management and communication skills
