Our Social Workers play an essential role in providing a positive experience for patients and their families by providing case and group work services during the hospital stay. The Social Worker acts as an advocate on behalf of the patients to ensure maximum access to quality healthcare. Responsibilities include: Functions in a consultative and educative capacity with all pertinent individuals or agencies that have an impact on patient care. Provide direct service to patients and their families by using a variety of social work modalities. Travel to patient homes and community meetings. Maintain appropriate professional patient care documentation as defined by hospital policies and procedures. Prepare materials for supervisory conferences. May direct the activities of paraprofessional staff, social work interns and volunteers. May participate in appropriate research, staff development programs and community liaison activities.
