Palliative Medicine Nurse Liaison — Lincoln County Job in Lincolnton 28092, North Carolina US

Work Location:  Lincoln County


Position Summary:
  Provides direct patient care to patients in the Palliative Care program, in collaboration with and under the auspices of the Palliative Care physician or Nurse Practitioner.


Education and Experience:
  Current, active, and unrestricted NC and SC license as Registered Nurse (RN).  Current CPR required; CHPN preferred.  Minimum 2 years related experience.  Experience in direct care of patients with life limiting illnesses required.  Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.  Recent hospital experience and ability to perform clinical assessments required.  ELNED training and experience with hospice and palliative care preferred.  Computer proficiency is required.


Knowledge and Skill Requirements:
  Excellent interpersonal and communication skills (both written and verbal).  Strong problem solving and critical thinking skills.  Well organized, self-directed and able to manage time.  Flexible, innovative, resourceful and creative.  Ability to interact with patients and family members in a varying psychosocial circumstances complicated by the presence of terminal illness.  Ability to be diplomatic yet assertive.  Knowledge of nursing principles, practices, and policies, and sound clinical skills.  Understanding of hospice and/or home healthcare policies, and procedures.  Strong belief in hospice mission.  Excellent ability to work within a team.  Excellent ability to coordinate and foster multi-interdisciplinary care.

Reasoning Ability:  Ability to deal with a variety of variables under only limited standardization.  Able to interpret various instructions.

Mathematics Ability:  Ability to perform basic math skills, use of decimals to compute ratios and percents, and to draw and interpret graphs.
 
Language Ability:  Ability to read a variety of books, journals, magazines, instruction manuals, regulations, interpretive guidelines, and instructions.  Ability to prepare memos, reports, and essays using proper punctuation, spelling and grammar.  Ability to communicate distinctly with appropriate pauses and emphasis, correct pronunciation (or sign equivalent), and variation in word order using present, perfect, and future tenses
