The primary responsibility of the Anticoagulation Clinic Supervisor is coordinating and managing the outpatient Anticoagulation Clinic. He/She promotes and manages anticoagulation therapy based on current clinical knowledge that is consistent with policies established at the local, regional, and national levels.
Clinical: The Anticoagulation Clinic Supervisor will provide clinical services primarily to outpatient Anticoagulation Clinic. Clinical services include, but are not limited to the following:
- participation in a multidisciplinary approach to provide quality patient care
- provision of drug information to healthcare professionals, patients, and patient representatives
- recommendation of appropriate treatment regimens or additional interventions to maximize patient care outcomes considering choice of therapy, safety, efficacy, and pharmacoeconomics
- assists in managing global and individual clinic consults, database management, and laboratory compliance of point-of-care monitors
- communication of local, regional, and national Veterans Affairs medical center initiatives to healthcare providers related to drug therapy
- documentation and report of adverse drug reactions through applicable programs and modification of patients treatment regimens as necessary
Education and Training: The Richard L. Roudebush VA Medical Center is a teaching institution affiliated with Indiana University School of Medicine as well as Purdue University and Butler University colleges of pharmacy. The Anticoagulation Clinic Supervisor serves as a preceptor for the departments PGY1 and PGY2 pharmacy residency programs and as a preceptor for final year pharmacy clerkship interns from colleges of pharmacy throughout the year. The Anticoagulation Clinic Supervisor develops and presents formal and informal educational inservices to healthcare professionals including pharmacy staff.
Administration/Supervisory: The Anticoagulation Clinic Supervisors administrative duties include, but are not limited to the following:
- immediately responsible for functions and activities of the anticoagulation clinic
- continually evaluates the clinical pharmacy program for opportunities for improvement
- completes job performance evaluations, recommendations for awards, and/or disciplinary actions for clinical pharmacy specialists, and makes recommendations to Chief of Pharmacy when appropriate
- participates in meetings, conferences, and training programs relevant to the development and maintenance of the clinical pharmacy department
- ensures compliance with established pharmacy policies and procedures
Program Management: The Anticoagulation Clinic Supervisoris responsible for the development, implementation, and monitoring of drug utilization programs, clinical protocols, and guidelines when necessary. He/She collaborates with other healthcare professionals to optimize the medication use process from all aspects including drug use evaluations, quality assurance programs, and systems redesign projects.
Required Skills:
Citizenship: U.S. Citizen (Non-citizens may be appointed when no qualified citizens are available in accordance with current regulations)
Education: Graduate of a degree program in pharmacy from an approved college or university. The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE), or prior to the establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP).
Specialized experience: Completion of the equivalent of 1 year of creditable experiencewhich is directly related to the position to be filled or completion of a 1 yearASHP-accredited Residency. Residency training and previous supervisory experience is preferred.
Licensure: Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e. Puerto Rico), or the District of Columbia.
