RN Position – On-Call
Full-Time Evening/Nights
Hours are 5p- 8a / 4 nights per week
Overview:
On Call RN is responsible for evening, night, weekend and holiday on-call duty, provides nursing assessment and interventions for patients with identified urgent needs and gives instruction and support as appropriate to caregivers.
Primary Responsibilities Include:
- Provides on-call coverage as scheduled.
- Responds to pager messages within 15 minutes; triages for making phone contacts and making visits at patients’ residence to provide direct care for urgent patient/family needs.
- Communicates with attending physician, Hospice IDG team and other agencies as needed to develop a comprehensive plan of care.
- Provides on-call coverage as assigned and participates in the back-up and on-call rotation.
Minimum Qualifications:
- Registered nurse, licensed by the State of Ohio, BSN/CHPN preferred.
- Three years clinical experience, some hospice or home health experience preferred.
- Excellent nursing assessment, interpersonal and communication skills.
- Ability to perform job duties independently, cope with patient/family emotional stress and be tolerant of diverse lifestyles.
- Knowledge and skills to provide care appropriate to the ages of patients and family member served in the ECF, ALF or home setting.
Other helpful skills:
- Computer literate, experience with word processing, Microsoft Excel, e-mail.
- Current driver’s license, required vehicle liability insurance and access to adequate transportation for agency-related travel.
- Ability to perform duties independently, cope with patient/family emotions stress and be tolerant of diverse lifestyles.
