Saunders House is seeking a seasoned and highly skilled RN Admissions Nurse.
The primary focus of this position is to performs accurate, thorough, and timely nursing assessments of patient/resident care needs in accordance with Nursing Department standards of practice and with consideration to age, mental, social, and physical status and response
Position Summary:
Provide skilled professional nursing care to patient/family/caregivers as prescribed by the physician and in compliance with regulations by the State Board of Registered Nursing, the patient and Company policies. Oversee and provide primary care, including all clinical care, palliative care, and symptom control and identify other physical, psychological, social and spiritual needs for assigned patients. Regularly assess the patient for nursing needs. Implement the Plan of Care Provisions to meet their needs, initiate appropriate intervention and support for the patient and family upon admission and re-evaluate the patient’s nursing needs to provide appropriate, comprehensive and responsive care plan interventions. Work closely with other members of the interdisciplinary team to ensure all patient and family care needs are being met. Adhere to Company policies and procedures and regulatory compliance requirements and directives.
Essential Responsibilities:
- Coordinates, assesses, plans, delivers, and evaluates nursing care for admission/readmission patients/residents
- Under direction of Unit Manager, provides leadership and direction for the delivery of nursing care by Licensed Practical Nurses (LPN), Nursing Assistants, and Unit Coordinator on assigned shift
- Follows and enforces strict adherence to safety and infection control rules, regulations, policies, and procedures
- Serves as a mentor and role model and represents Saunders House in a manner that conveys professionalism, appearance, confidentiality, courtesy, conduct, honesty, fairness, personal integrity, and a respect for the fundamental rights, dignity, personal comfort, and privacy of others.
- Performs and/or actively participates in special research assignments, performance improvement processes, and special projects, as required or as assigned. May serve as a member or participant on various internal and external committees
- Manages and coordinates other related nursing and management responsibilities as necessary. Remains flexible and adaptable in work schedules and work assignments as defined by departmental and facility needs
Education Experience:
Graduate of an accredited Nursing program required. Licensure and registration by the Pennsylvania Board of Nurse Examiners as a Registered Nurse required. Prior experience in a skilled nursing care facility, acute care setting, or hospital is required.
