RN CASE MANAGER an INNOVATIVE NURSING POSITION We are currently recruiting an experienced geriatric RN who is interested in a non-traditional philo- sophy of health care out of our McKeesport center. This is a Full time Monday - Friday daylight position serving a frail population whose goal is to continue living in their own home. The RN Case Manager provides the skills necessary to efficiently assess the need for, and coordinate the delivery of, nursing care and home / personal care. We are seeking candidates with at least 1 year geriatric and home care experience, the ability to prioritize, be flexible, and be a team player.
