Social Worker
Nemours is seeking a Social Worker (START DATE AUGUST, 2012) to join our team at the Nemours Children's Hospital in Orlando, Florida.

Description
Provides
expert social work evaluation and treatment services to patients and
families with complex psychosocial needs independently. Acts as a team
Lead in coordinating services with the interdisciplinary team and
community agencies to ensure appropriate patient care across the
healthcare continuum. Coordinates and/or provides clinical supervision
to members of assigned social work team. Demonstrates commitment to a
patient and family-centered model of care.
The Lead Social Worker
will perform the essential functions listed below as well as serve as a
resource for Social Work and Mental Health colleagues with Nemours
system navigation and clinical decision making related to patient and
family clinical care, multidisciplinary care and inpatient care.
Clinical Care
- Manage
complex assessments of the patient / family's bio-psychosocial
situation and establishes goals and periodically re-assesses the
pt/family situation. Documents a complete psychosocial
assessment/history of patients and families - Provide
crisis intervention services, therapeutic / goal directed counseling
for patients and their families while assisting them with emotional,
personal, psychological, socioeconomic factors, and adjustment to
illness issues, to include the entire spectrum from diagnosis throughout
the treatment course. - Serve as facilitator of support, therapy and educational groups in relation to patient population served.
- Assess, treat, and/or triage referrals for Counseling and Psychiatry (internally or to external community providers).
- Continue
to update education and level of skill in job area. Attends in-service
presentations, and continuing education programs to maintain and
increase knowledge base.
Multidisciplinary Care
- Attend patient care unit and/or Medical division rounds in accordance to team expectations and provide
- inpatient admission screening on assigned patient care unit.
- Establish
and maintain effective working relationships across disciplines and
coordinate family meetings. Coordinate recommendations of team meetings. - Participate
in the interdisciplinary assessment and treatment of patients and their
families, and provide consultation for physicians and other members of
the health care team to determine current psychosocial needs. - Coordinate and facilitate / co-facilitate clinical team meetings.
Inpatient Care
- For
NCH admissions, the Social Worker will interface with insurance
providers, case managers and others involved in the authorization of
needed patient services. Identifies and negotiates differing needs
within the family system. - For
NCH admissions, the Social Worker working weekends will coordinate
discharge planning needs (in conjunction with the Clinical Nurse
Specialist and discharge planning Nurse Navigator) of patients and their
families in an efficient, effective and timely manner while including
the family in the process. - When
on-call, respond to on-call events and requests for service within 5
minutes of receiving page and remain within a 30 minute drive of
hospital. - Act as liaison between hospital and the various community agencies, which provide aid to families and children.
- Document social work notes in the patient medical record within the guidelines of the Department policy.
- Keep appraised of community resources and provide resource specific information to the assessed needs
- Maintain a professional manner at all times with staff and family.
Requirements
- Masters degree in Social Work from a program accredited by the Council on Social Work Education (CSWE)
- 1+
years experience - Graduate school internship may be applied to the job
related experience at the discretion of the Senior Director. - Must have advanced ability to document clinical notes in the EMR.
Must be flexible to work some weekends
