LVN
As an LVN, you will: Work with Registered Nurse Case Manager to provide home health. Visit the home health patient according to the plan of care. Assess individual status and care / instruction needed by the patient. Provide hands-on care, and communication about the patient to the case manager. Has the autonomy to make individualized patient-centered decisions for optimal clinical care. Works with other highly skilled clinicians through a multi- or interdisciplinary team approach.
