The Transition Planner is responsible for coordinating and integrating activities across multiple functional lines, and managing stakeholder communications. The Transition Planner accomplishes the above by managing project scope, time, cost, and quality. Finally, the Transition Planner applies project management, general management and technical skills, as well as team management, negotiation, financial and business acumen, combined with an understanding of organizational politics to meet project objectives and to meet or exceed stakeholder expectations.

·       Schedule and hold Kick Off Meeting

 

·       Set up Interview Schedule (List of Interviews, Dates / Times)

 

·       Conduct Interviews

 

·       Conduct a Facility Tour

 

·       Request data from the client

 

·       Analyze data

 

·       Develop organization chart for Transition Teams and their integration with Project Management Team.

 

·       Develop Transition Steering Committee

 

·       Develop Departmental Leadership Group

 

·       Develop Facilities Readiness Team

 

·       Develop Operational Readiness Team

 

·       Develop Communications Team

 

·       Outline the objectives, roles and responsibilities for each Transition Team

 

·       Set up Transition Teams meeting schedule

 

·       Develop a Master Transition Schedule in Microsoft Project

 

·       Pull Schedule Session #1

 

·       Pull Schedule Session #2

 

·       Define components of Transition Budget with the internal Transition Team

 

·       Assign costs to Budget Components

 

·       Validate costs with Financial Departments

 

·       Develop a Master Transition Budget

 

·       Integrate with Project / Program Budget

 

·       Develop Analysis and Recommendations Matrix based on data review and interviews

 

·       Finalize Analysis and Recommendations Matrix

 

·       Schedule first meetings with each Transition Team

 

·       Review the Analysis and Recommendations with each Transition Team

 

·       Update budget with all expenditures and manage the Transition Budget

 

·       Define roles and responsibilities for each member on the Operational Readiness Team

 

·       Manage items on the Issues Log

 

·       Coordinate the development of the Move Manual

 

·       Coordinate development of updated policies and procedures

 

·       Outline operational assumptions in the basis for design and incorporate these into the development of the Move Manual

 

·       Identify key department adjacencies affected by the move

 

·       Manage the development of orientation and training plans and add to the Move Manual

 

·       Develop Schedule for Mock Moves (if applicable) and add to the Move Manual

·       Finalize Move Manual

·       Manage Department Specific Day of Move checklists

·       Manage Day of Move Roll Call

·       Define roles and responsibilities for each member on the Facility Readiness Team

·       Manage items on the Issues Log

·       Confirm that an effective plan is in place to integrate and activate systems

·       Assist in the development of the Move Manual

·       Coordinate inspection and licensure

·       Coordinate building commissioning activities

·       Revise/Develop Emergency Plans

·       Coordinate Department of Health Inspection (if applicable)

·       Ensure client receives Certificate of Occupancy (if applicable)

·       Manage House Wide Day of Move checklist

·       Work with the Project Manager to manage the Punch-list prior to Occupancy

·       Coordinate schedule for delivery, installation and relocation of medical equipment, fixtures, furniture and other equipment

·       Facilitate the development of a technology integration and activation plan

·       Create an FFE and Technology responsibility matrix

·       Work with relocation specialist to establish move procedures

·       Manage the development of the Command Center team and their role on Move day

Bachelor’s Degree Required in one of the following areas: Construction Management, Healthcare Administration, Industrial Engineering, Engineering

Certifications: PMP and LEED (Green or AP)

6 years of experience in healthcare Transition Planning/Project Management required

Travel within the U.S. will be required
