Care Management Coordinator
Job Duties Include: Primary responsibility is day to day coordination of patient care, communication, of patient data to appropriate personnel, receiving initial referral information and determining appropriateness for home care. Works effectively as a team member to ensure referral and insurance data is entered into the data base accurately. Serves as Case Manager for private and managed care patients. Receives initial referral information and collaborates with Homecare Assessment, nurse, pharmacist, physician, and appropriate staff prior to initiating care. Serves as liaison between the agency, nursing staff, and external parties regarding patient care. Maintains working knowledge of Walgreens Infusion and Respiratory Services contracts and negotiates fee for service. Coordinates, copies, and delivers referrals to pertinent staff (Pharmacy, Billing) in a timely manner. Performs as a clinical resource to staff, field nurses and families. Demonstrates commitment, professional growth and competency. Adheres to all company policies and procedures. Minimum Requirements: Graduate of an accredited school of nursing. Registered Nurse (RN) licensed in the current state of practice. Valid and current driver's license and evidence of automobile insurance. Flexibility and ability to work effectively within a multidisciplinary team. Ability to communicate effectively (verbal and written) with team members, patients, caregivers, physicians and referral sources. CPR Certification. Ability to solve practical problems and deal with a variety of variables in situations where only limited standardization exists. Ability to define problems, collect data, establish facts, and draw valid conclusions. Minimum of 3 years relevant nursing experience with at least one year in home health care. Must possess strong clinical skills and outstanding interpersonal and organizational skills. Preferred Qualifications: Bachelor of Science degree in Nursing. Previous infusion experience. Pediatric background preferred. Knowledge of general office procedures and equipment is preferred.
